Things that can lead to this kind of error are unclear prescriptions that a pharmacist did not clarify, a doctor’s incorrect diagnosis of a problem or an incorrect combination of medicines that can lead to devastating results.

ATTORNEY JACK D. RAY: Medication errors are unfortunately still a common problem in medicine today. We all have seen pharmacy prescription slips that are very difficult to read and unfortunately they can sometimes be difficult for pharmacists and others to read. And in those circumstances where it is unclear, clarity should be brought to the situation by confirming the prescription with the doctor’s staff. Sometimes the physician or other healthcare provider himself is responsible for the error by prescribing an incorrect diagnosis or an inappropriate combination of drugs that can have devastating results.
